ConclusionsAlthough adjuvant therapy for all patients may represent overtreatment, the findings of the present study suggest that, in the interest of fertility preservation, continuous postoperative hormonal treatment should be administered, at least to patients younger than 32 years. In patients who decline hormonal treatment, we recommend that they undergo follow-up for recurrence until 5 years after surgery.

“Introduction: Since the phenomenon of hyperfiltration in primary aldosteronism (PA) was first noted in 1996, subsequent clinical studies have produced conflicting results. To determine the development of relative hyperfiltration in PA, we performed a meta-analysis.

Methods: MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials (CENTRAL) were searched through to July 2009. Reference sections of original articles, meta-analyses, and reviews on hyperfiltration in PA were reviewed. Hypertensive

patients provided the controlled data for hyperfiltration. Two authors independently extracted the click here data.

Results: A total of seven studies were included. One study was from the data of the TAIPAI group. Overall, there was strong evidence that relative kidney hyperfiltration existed in PA (fixed-effects model: standardised mean difference (SMD), 0.13; 95% confidence interval (CI), 0.03-0.22, p = 0.007; random-effects model: SMD, 0.35; 95% CI, -0.01-0.71, p = 0.05), though with a significant heterogeneity (p < 0.0001). In the secondary meta-analysis with five top-quality studies, the relative kidney hyperfiltration was more significant. Mean age in each enrolled study was the only factor significantly associated with the existence of heterogeneity among the selected studies in the meta-regression analysis.

Conclusions: Current evidence suggests that relative kidney hyperfiltration is the hallmark in PA and the phenomenon is beyond the effect of hypertension of PA. Clinicians should be aware of the possibility of occult renal damage in patients with PA.”

“SETTING: Douala, the economic capital of Cameroon, with a network of 20 diagnostic and treatment centres for tuberculosis (TB).

OBJECTIVE: To describe the spatial distribution of smear-positive pulmonary tuberculosis (PTB) cases in Douala, Cameroon, and to evaluate links between PTB incidence and

patients’ socio-economic status (SES).

DESIGN: Between May 2011 and April 2012, demographic clinical characteristics and global positioning system coordinates find more for the residence of each consenting PTB case were collected. Spatial scan statistic was used to identify spatial clusters of tuberculosis. SES data were collected by interview during home visits. PTB patients’ SES data were compared with those available for the whole urban population from a recent demographic and health survey. The correlation between PTB incidence and the mean SES of the PTB patients was examined by health area (HA).

RESULTS: In total, 2132 (84%) diagnosed PTB cases were identified and interviewed. Three statistically significant spatial clusters were identified. High PTB incidence was predominantly found in HAs characterised by low SES.

CONCLUSION: There is evidence of clustering of PTB cases in Douala. Clusters are mainly found in neighbourhoods of low SES. Systematic use of cluster detection techniques for regular TB surveillance in Cameroon might aid in the effective deployment of National TB Programme resources.

contraction stress (CS), and degree of conversion (DC) of four dimethacrylate-based and one silorane-based composite were investigated. To determine shrinkage, the composites were packed, respectively, into a cylindrical cavity in human teeth and imaged using X-ray microcomputed tomography to determine the precise volume before and 30 min after photopolymerization. To determine CS, the sample was applied in a similarly sized cylinder in a universal testing machine and monitored for 30 min. FTIR spectroscopy was used to determine DC. The volumetric shrinkage (range: 1.1-3.1%) and maximum CS (range: 1.2-3.5 MPa) differed significantly among the tested composites but not the final DC (range: 62.3-69.1%). The silorane-based composite displayed the lowest volumetric shrinkage and CS of all composites. No correlation was observed between the stress and volumetric shrinkage values of the dimethacrylate-based composites. A moderate correlation was found between stress and DC (r = 0.836), which was significant at 20 and 40 s.
